This certificate is issued for the workers who deal with foodstuff and activities related to public health to verify their good health and free from any contagious disease.

Procedures of application

• Before starting the work, the worker should apply for a health card from Health department and pay the prescribed fees at Municipality Treasury.
• Two photos should be presented plus a copy of the passport or the Identity card.
• The fees should be paid at Municipality Treasury and the receipt should be presented to the Follow-up section at Health Department.
• The form of Health cards should be filled and transferred to the precautionary medicine At Ajman medical region to finalize the procedures of Health examination and card authentication.
